In 1996, Congress passed and the President signed the Health Insurance Portability and Accountability Act-HIPAA. This legislation included a provision for health care providers to develop procedures and policies to assure protection of your personal health information.
The physicians and staff at Dubuque Internal Medicine, P.C. have always respected the importance of maintaining your health information confidentially. HIPAA requires that we notify you of the privacy practices currently in force at Dubuque Internal Medicine, P.C. This legislation also provides you with certain rights regarding your medical records. These rights are outlined in our NOTICE OF PRIVACY PRACTICES (attached).

* your rights, including your right to access your medical record, your right to amend your medical record, your right to an accounting of disclosures we make and your right to receive communications by alternative means.

* how to submit a complaint if you believe your privacy has been violated.
